Federally Qualified Health Centers
We understand the unique needs of Federally Qualified Health Centers (FQHCs) as they navigate the transition from fee-for-service to value-based care. From HEDIS to UDS, our platform helps you achieve vital population health improvements while streamlining short-term operational goals.
Our work with FQHCs began in 2014 and has scaled to serve 1 in every 7 health centers across 40 states.
Free and Charitable Clinics
We support Free and Charitable Clinics and Pharmacies, the organizations on the frontlines of delivering care to the communities with the greatest needs. Our technology is designed to be flexible, adjusting to the unique operations of clinics of all sizes, from those open once a week to those with daily schedules.
Since 2013, we’ve grown to partner with 1 in every 6 Free and Charitable Clinics nationwide, continuously bringing them solutions to maximize their reach and impact.
Tribal Health Organizations
We are honored to partner with Tribal Health Organizations and Urban Indian Health Centers. We support their operational and clinical goals, helping them deliver culturally resonant population health improvements.
Our work in this space began with a pilot funded by the Indian Health Service, and given the success of these early pilots, we are committed to expanding our support for advancements in health outcomes for American Indian and Alaska Native populations.

Strategic partnerships with national associations like the National Association of Community Health Centers (NACHC) and the National Association of Free and Charitable Clinics (NAFC) allow us to support entire segments of the safety-net. Together, we generate crucial insights, advocate for policy change, and champion the needs of providers and patients nationwide.
State & Primary Care Associations
We work with state and primary care associations (PCAs) to implement statewide solutions and approaches that directly impact the health of entire regions. Together, we define and deploy multi-pronged strategies proven to outperform UDS and payer-defined benchmarks. Our collaboration helps their member health centers be successful in value-based contracts.
Clinically Integrated Networks
We provide Clinically Integrated Networks (CINs) with flexible patient segmentation and outreach tools to help them go beyond standard reimbursement models. Our platform helps fine-tune outreach to specific populations who need it most, supporting advanced value-based care initiatives.

We also build unique cohort-based programs designed to turn patient conversations and behaviors into insights and high-performing workflows. Our collaboration helps their member health centers leverage technology more effectively for population health.

Electronic Health Records
We integrate with leading Electronic Health Record (EHR) systems to manage patient segmentation and automate follow-up, particularly in areas where EHRs fall short on patient engagement tailored for low-income populations. Often, we bridge the gap between operational efficiency and health outcomes.
