Patient outreach is a critical component for every healthcare organization— especially as the industry continues to transition away from fee-for-service models in favor of value-based care. For FQHCs and community clinics responsible for treating underserved communities in urban and rural areas, the challenge to maintain effective communication becomes much greater. Patients in underserved communities are often significantly impacted by multiple social determinants of health, which prevent them from receiving adequate healthcare or the tools necessary to make good health choices on a daily basis. In order to manage patients and empower them to prioritize their health, organizations should carefully consider the efficacy of the vehicles used to reach these populations, as well as the content and style of the communications.
