Human Telephone vs Text Message Counseling and Physical Activity Among Midlife and Older Adults: A Randomized Clinical Trial

Summary

This study compared the effectiveness of a 12-month walking program delivered by human phone advisors versus an automated text messaging program. For inactive Latino/a adults, the customized program delivered through the CareMessage platform was found to be equally effective at increasing physical activity and improving well-being.

Key Findings

  • Computer-driven text messaging programs can achieve walking increases comparable to trained human counselors among underserved Latino adults over 12 months.
  • Primary Results:
    • Human advisors: 116.4 additional minutes of walking per week
    • Short Message Service (SMS) program: 113.6 additional minutes of walking per week
    • Difference: Statistically equivalent (2.8 minutes difference)
    • Both groups: Significant improvements maintained for full 12 months
  • Secondary Results:
    • Both programs helped 24-31% of participants meet national physical activity guidelines (150+ minutes/week)
    • Participants maintained weight over 12 months (avoiding typical annual weight gain)
    • Reduced sedentary screen time in both groups
    • Improved overall well-being and quality of life

Clinical Significance

This research demonstrates that automated, customizable text messaging programs, like those powered by CareMessage, are a viable and effective alternative to traditional human counseling for promoting physical activity. This technology-enabled approach can significantly increase access to evidence-based health programs for diverse and underserved populations.
